Does your family need services to help a child with a developmental disability?
If you have a child with special needs, your family may qualify for state services (unrelated to any services you may or may not currently receive from District 204). To access state services, you must register in the statewide PUNS database maintained by the Illinois Department of Human Services (IDHS). Enrollment ensures services are available to you and your family when you need them...whether you need services immediately or anticipate needing services in the future. Illinois state government agencies are also using enrollment as a gauge for needs assessment, program services, and funding.

Contact the agency nearest you to complete a "Prioritization of Urgency of Need for Services" (PUNS) form. Agency locations are provided in the brochure.
